Objective: To evaluate the early outcomes of minimally invasive mitral valve replacement surgery (MIMVR) via right mini-thoracotomy at Phu Tho Provincial General Hospital. Methods: A retrospective descriptive study was conducted on 40 patients who underwent MIMVR between June 2023 and May 2025. Postoperative outcomes were assessed at one-month follow-up. Result: Mean age was 57.8 ± 10.4 with females acounting for 60%; pre-operative NYHA class III-IV in 45% (18 cases); 5% (2 cases) had a Left Ventricle Ejection Fraction below 50%. The pathogenesis of rheumatic heart disease 85%, degeneration 15%. The mean cardiopulmonary bypass time was 113.8 minutes and the mean aortic cross-clamp time was 70.6 minutes. Conversion to sternotomy was required in 2.5% (1 case). The mean mechanical ventilation time and hospital stay were 6,6 hours and 9 days respectively. Conclusion: Minimally invasive mitral valve replacement surgery via right thoracotomy is a safe, effective and seasible method with favorable early outcomes in a provincial-level cardiac center.
