Abstract Solitary and pelvic kidney is considered a rare case in urology. It may accompany with ureteropelvic junction obstruction (UPJO), which is even rarer in daily practice.We had a 40-year-old male with left solitary pelvic kidney with UPJO corrected in other center before. He presented with signs and symptoms of acute pyelonephritis. He had a double j ureteral stent that is replaced every three months due to recurrent hydronephrosis. Treatment was conservatively by intravenous antibiotics, with insertion a new double j stent.
